In consideration for the Thunderbirds Waterpolo agreeing to provide a position on an age‐group team for the athlete(s) listed below, you confirm that you have read and understand the club’s regulations and agree to comply with them all. Specifically, you agree to accept financial responsibility for the full fees of the athlete(s) you are registering. You have provided below a credit card, in good standing, and you give the Thunderbirds Water Polo Club permission to charge your credit card for any monies you have outstanding to the club.
Fundraising and Volunteer Obligation Contract
As members of the Thunderbirds Water Polo Club, parents are expected and obligated to participate in fundraising efforts throughout the year, primarily through casinos, raffles, and various other fundraisers.
If the AGLC assigns our club a casino during the year, parents will be obligated to work a casino shift.
Along with fundraising requirements, parents are expected and obligated to volunteer as minor officials at all Community League, Provincial League, AWPA sanctioned, and Water Polo Canada sanctioned games. Parents are obligated to assist with all club operations. You will provide a credit card, in good standing, to the Thunderbirds Waterpolo Club and you give the Thunderbirds permission to charge your credit card an amount determined using the table below for any missed or shortfall of fundraising and volunteer obligations.
Casino Shift $300 for not working or missing a casino shift.
Other fundraising initiatives $150.00 for not fulfilling fundraising requirements.
Minor Officiating and assisting with club operations. Refer to the registration fee chart.
Personal Information Agreement
You understand that the Thunderbirds Water Polo Club collects personal information about each of its registrants, including name, address, email(s), telephone number(s), sex, age, and date of birth. This information is used for the purposes of club communications and coordination. The information is also used by and disclosed to the Alberta Water Polo Association (AWPA), Water Polo Canada (WPC), and Sport Canada for annual demographic reporting, registration and rosters, age group determinations, and to communicate with participants about water polo programs, events, and activities. Medical information you provide will be given to all team coaches and in the case of team travel, to the assigned trip chaperone(s).
You understand that by registering, you consent to the use of your personal information by the Thunderbirds Water Polo Club for its operations. Furthermore, you confirm you will not use personal information disseminated by the Thunderbirds for purposes unrelated to club functions.
Code of Conduct Agreement
You acknowledge and certify that you and your athlete(s) will conduct yourselves in accordance with the Thunderbirds Code of Conduct. Copies of the Swimmers Code of Conduct and Parents Code of Conduct attached. You also acknowledge and certify that you and your athlete(s) will conduct yourselves in accordance with the Alberta Water Polo Association and Water Polo Canada’s codes of conduct.
Respect In Sport
Alberta Water Polo is requiring that all families have at least one parent take the Respect in Sport online training. This online certification costs $12.00 (valid for five years). Please ensure that you complete this course prior to registration going live as you will need to upload your certification number in order to complete your online registration with Thunderbirds. If you have taken this training for another sport you can upload that certification number and do not need to redo the course if it was completed in the last 5 years.

Liability Release Agreement
You acknowledge that the Thunderbirds Waterpolo Club assumes no liability for personal injury, damages, or loss of personal property while involved in or associated with any club activity. Furthermore, you hold club members, coaching staff, officers, or duly authorized persons harmless from liability and hereby authorize the Thunderbirds or its representatives to take actions they deem necessary to correct, or attempt to correct, any situation which may result in personal injury, property damage, or loss of personal property while involved with any club activity.
All fees are due at the time of Registration (both fall and winter) or a payment plan may be considered of 50% at the time of registration, 25% of balance November 1, and the final 25% January 1.
Failure to pay fees on time as per the schedule defined above may lead to the member being held as NOT in good standing and may result in sanctions as determined by the board which may include but is not limited to athletes being withheld from participation in club practices and events.
In the event that a refund is requested from a member in good standing,
50% Refund for withdrawal before October 15, minus Fundraising obligations
30% Refund for withdrawal before November 30, minus Fundraising obligations
15% Refund for withdrawal before January 1, minus Fundraising obligations
No refund after January 1 and beyond.
In the case of extenuating circumstances, the Board will consider on an exceptions basis if any refund and the amount thereto is provided.
Notice of withdrawal must be received by the Registrar and Treasurer prior to the dates above in writing by email. Verbal notification to any Board member or official is insufficient notice for purposes of a refund to be issued.
